We put a big emphasis on safety and comfort, so want to make sure everyone is fit to dive, before their arrival.

*Please note if everything is ‘No’ you are not required to print the form or bring anything with you.

*If you believe you have a “Yes’ on the medical, again feel free to contact us first, a confirmed ‘Yes’ would mean you need sign off from a physician before partaking in any diving activities.
